How many feats at 3rd level?

I'm really puzzled by this suddenly. I read the PHB for a 3rd level dwarf fighter and I count 3 feats for a 3.5 dwarf. 2 at 1st level and a bonus at 2nd. Then I cranked out an npc from etools and bingo! it shouts out the guy has 4 feats

Who's correct? and why?

Every character gets a feat at 1st level
Every character gets a feat at 3rd level
Fighters gain a bonus feat at 1st level
Fighters gain a bonus feat at 2nd level.

Total: 4 feats.

It's four. Base feat at first, bonus fighter at first, bonus fighter at second, another base feat at third.
