Move though an enemy space?

Only thing I've seen that accomplishes this is an epic feat, and if the size difference is great (don't remember exactly how great).

Yup, Flanking Maneuver is an Epic Feat that allows you to move diagonally across squares containing obstacles, and through squares occupied by enemies. Can't end your move in their space though.

Also, you can move through an opponent's space if it is two sizes larger or smaller than you. You provoke OAs as normal.

In 4e you can slide or move through the corner of an enemy's space. If you slide, you do not provoke. You cannot move through the corner of a space that has a pillar or statue or some similar permanent structure.

Warlord can shift willing allies iirc. Not sure about through occupied squares.

Actually it says "... most creatures," probably to allow DMs with Gelatinous Cube issues a small fiat.

And yes, forced movement in the form of a push, pull, or slide does not provoke OAs. An interesting tidbit - if you use forced movement to drop somebody into a pit, acid pool, etc. they get to make a save at the precipice. If they pass they fall prone at the edge, if they fail they are moved the rest of the way. :D
