I am not fond of the Far Realm. I think anything like that should be within the realms of the gods. Besides if they map out what exactly is the Far Realm someone else will say that beyond that is the Extremely Far Realm where even aberrations fear to tread.

New Setting

I am pretty confident that they will come up with a new setting.

Ravenloft is too convenient for online articles with its "one domain per article" construction process, and it's been mentioned in the Manual of the Planes.
