I put in Action Points in my next campaign specifically to limit this. I know in the last campaign we played in, we would have seen at least 3 PC deaths in 5 sessions, all against level appropriate encounters, if it hadn't been for Action Points saving them. </p><p></p><p>I agree that combat should be uncertain.
