Expertise can be invaluable to this type of build. Essentially you take max Expertise to boast your AC until you tumble into sneak attack postion, unleash the sneak attack bomb and then tumble out, rinse and repeat. It can be quite effective. I also multiclassed as a Ranger to get Two Weapon fighting, for free and the extra HP,(this was 3.0 version). I had stats similiar to yours, I would reccomend swaping your Con score for your Cha score, with the skill ranks you will have you will not notice the loss of bonus for social skills. You are going to be wacked, and HPs are definetely the draw back to the build, though it is always exciting. If you have a hang back cleric have him or her cast Shield Other, and all the AC and HP boosting spells on you. I had a high Con Barb/Cleric in the party to cast that on me, and share the damage coupled with a monk in the party to setup flanking situations. Team work is critical for this build.
