D&D 3E/3.5 (3.5) Lethal Grappling?

Don't forget Warlocks with Grappling Blast - allows you to channel Eldritch Blasts on successful grapples. Would stack up with Black Blood Cultist very well.

So, side question. Are there any real rules as far as trying to choke an opponent to death during a grapple or breaking an arm or neck during a grapple?
 

You can assume that such acts are part of dealing unarmed damage during a grapple. The flavour of "how" is up to you, so long as the damage dealt remains the same.

For instance, 10 points of damage could occur from a punch to the face as easily as it could occur from blocking blood flow to the brain for a moment or so.
D&D doesn't have a very good method of causing and tracking injuries.
