It has been on Zakhara for a year or two now, located inland of the Pearl Cities. You can see its current location on this map (a zoomed in snapshot of the one I linked on the first page of all the AQ threads). Rob, the cartographer, included it here and I'm going with his placement; "Qaybar" is originally mentioned in DUNGEON #63's <em>Blood & Fire</em> adventure as a human town with no description - a generic Arabian-esque settlement that serves little more than being a plot hook / starting point.
