This sheet was inspired by the player mats that many european boardgames use for tracking game information. It uses pennies, glass drops, or other tokens to track many of the conditions that change rapidly during combat.
After printing it out, fill in the large bubbles with the base values. Glass drops work best for moving along the tracks since you can see through them. For the hit point and surge section you can stack pennies on the tens place spot. For the condition list, I use drops for when I have a condition and keep a printout of the rules handy (http://www.enworld.org/forum/showthread.php?t=234182).
