Here's an idea. There's a creature in Open Grave... ah yes, a Rotvine Defiler.
Open Grave pg 130 [Religion] DC 26: A rotvine defiler is the profane remnant of an immortal devoted to nature or agriculture. The corrupted immortals were slain and sealed under the ground, where the seeds of evil caused them to return to life and outwardly manifest their malevolence. [Religion] DC 31: A rotvine defiler arises when a creature makes a sacrifice over the monster's earthly tomb, breaking the seals containing it. The creature usually retains none of its original intelligence or memories.
It's a level 26 Solo. My idea is that the river has been turned to blood from all the sacrifice that the warlock is doing on the rotvine defiler's "earthly tomb" in order to break the seals. The "river of blood" isn't actually tons of blood, it's just gone insane with necrotic energy. You can use other monsters like Offalians, Blood Aminotes, scavengers that feast on the dead, etc. around the river. The warlock has some allies of his own, especially devils - though he's not picky. You might want to add some kind of potential ally - I'd go with some kind of ancient smart spider, to play off the elf/drow thing - who has access to the seals that held the defiler in place. This thing is ancient, totally bored of life, and thus cares little if it dies; it doesn't much care if the defiler kills it. If the defiler is unleashed, I imagine the PCs will die. Make it clear that the PCs have "lost" - the defiler goes wild, killing thousands of Arkhosian soldiers until an elder dragon sacrifices itself to kill it - but don't run an encounter with the thing.
