The party is trapped in the pocket dimension with Shelatraox and an absurdly powerful demon. Shelatrox has gone completely insane because he has been trapped in the pocket dimension for several times what his normal lifespan should have been. The catch is that no one can leave the dimension until Shelatrox dies. When Shelatrox dies, the dimension (which is linked to his life force) fades away. This will release the PCs, but it will also release the demon. The demon is forbidden from killing Shelatrox for some special reason. Perhaps Shelatrox tricked the demon into a "lifetime of service" or some such. Or perhaps any attempt by the demon to inflict pain on Shelatrox actually inflicts pain on the demon by some spiritual bond. Quandry: do the PCs kill the insane guy who has lived seveal mortal lifetimes to free themselves? are they still willing to do this knowing it will release the demon? Bonus: if released the demon can become a pesky thorn in the PCs back, ploting all kinds of evil that the party need to put an end to. Bonus: the demon could also be a PC ally from time to time, since he "owes them" for releasing him. I know these two ideas are opposed, but that's where the fun is. The demon promises not to hurt the PCs and come to their aid ... say once per character ... but the goes on his own business enslaving other races or destroying villages or whatnot, putting him at odds with the PCs. The demon should be *very* smart. His deals should be carefully worded, and then his words should be twisted as much as possible.
