perhaps i'd have an example of one filled out at the beginning or on a web site. but the log itself would just be a blank page.

let the user fill in whatever they think is right. let them use their imagination.

Seriously. How can you make an adventure record that isn't overly redundant and meaning at the same time? First things first, lets keep it to one sheet. I doesn't have to look like the adventure record of yore, but has to serve a function. That is, to keep track of the events of an adventure in a simple, non-complex way.

Remember, this is a sheet for kids (ages 13-16). Currently they are using prerolled characters.

I guess I don't see exactly what you are asking here. There are hundreds are other ways to hand out information to players and if you are looking for them to have their own record sheets, the 'goldenrod' version that was mentioned earlier is a much better version.

These were used specifically as part of the Permanent Record Folder (as stated above) and without it, these are pretty much useless for a player to keep information on. Do a search for 1st edition/2 edition player character sheets and you'll find a host of better options.
