[Addictions Survey] Help, please!

I am taking a course about addictions, and need 10 respondents to a questionnaire about addictions. It is a self-administered survey of 30 questions (10 about alcohol use, 10 about illicit drug use, 10 about gaming). It should take no more than 15 minutes to complete.

I would be really happy if I could get 10 students under the age of 18 to respond, but I'll take what I can get! If you are under 18, please make sure you have parental permission to submit the data.

The survey is anonymous and respondents can not be identified. The findings will be reported in ways that ensure complete confidentiality.
Because I am interested in the use and non-use of drugs and process addictions, there is no assumption that anyone who completes the survey has ever used tobacco, alcohol, or other drugs or partake in the activities mentioned therein.

If you are willing to take part in the survey, please fill out the attached survey and email me the results as an attachment at denise@ambient.ca

PLEASE DO NOT INCLUDE ANY IDENTIFIERS IN THE .TXT FILE other than the ones asked for (age, gender, grade, marital status, and country). PLEASE DO NOT DISCUSS YOUR RESULTS WITH ME: I really don't want to know who my respondents are.

When I get home from visiting my mother this Friday evening, I hope to open up all the files in my attachments directory (without looking at the email account to which they're attached, which will be immediately deleted) and collate the data so I can hand in the assignment on Monday. Let me know if anyone's interested in the results (of the gaming questions). Thanks in advance!

A few clarifications on the questions

Whoops... Just thought I should have been more specific in the questions with regards to time role-playing.
This refers to time you set aside specifically for the purpose of role-playing dice and paper RPGS (as opposed to computer "RPGs") and includes snack breaks, divergences from game play, etc. Time preparing games, blue-booking characters, updating character sheets, sketching your character: these all count.
 

To protect the survey's anonymity, I won't say whether or not I even responded, but at least one of the questions is a bit leading - it's in the drugs section and asks (roughly) 'Can you stop using once you start?' Unlike the similar questions in the other two sections (which ask 'how often have you been unable to stop'), that one has the implicit assumption that you do use drugs, because you can't honestly answer it if you don't (there being a lack of 'don't know' or 'not applicable' choices).

Just thought I'd mention in in case the methodology of the survey is important.

Please do NOT exclude caffine from addictions. The is a HUGE level of addiction to caffine in the western world that is ignored far too much.

Thanks for the comments. I had to translate the drugs section, and forgot to preface the comments with "if you do you or have used drugs"...

I agree that there is a problem with it, but since it is not illegal and the physical and psychological problems are less acute than other illicit drugs, I excluded it.
 

I'm also not goign to say if i responded or not...

A question tho. It seems that the survey on drug use is misleading. All the alcohol questions are 'in the last year' and none of the drug ones are. If you're comparing the two sections it's going to give you skewed data surely? I mean I could have been a heroin adict 20 years ago and not touched any illegal drug since then.

Also the question about illegal activities to do with drug use. maybe i'm nitpicking, but surely you have to engage in illegal activities to get drugs. I mean holding and buyign them is illegal right?
