So PCs are in the abyss, and a hezrou utters a blasphemy.

For those that fail there save: where do they end up? On the prime material but where? All scattered different places? Their birthplace? The place they last left the PMP? Is it a forced plane shift (5d100 miles?)

I don't think the rules say, pick one and be consistant.

Though the place of their birth sounds nice and mythic and could lead to some great scenes.
