Combat Location, Stunts & Special Effects

I used to have Mastering Iron Heroes, which had awesome rules for stunts and special effects based on the location of the fight. Things like tossing bar stools at people, rolling overturned keg barrels to trip opponents, swinging on chandeliers or tapestries, etc.

I'm curious, what kind of effects have people done and more specifically, what kind of game effects have you imposed with these?

I was originally thinking 4E when I had this question (like does such-and-such maneuver knock people prone, immobilize them, weaken them, etc.), but I'm curious what other games have for effects too.

Just to offer a little of my own ideas: I used the suggestions in Mike Mearls' article about Solo Fights in 4E during my Players' foray into the Kobold Hall (DMG 4E) against the White Dragon. I had

*icy stalactites that could be knocked down to impale (did damage and stunned the person for 1 round),

*pools of water that could flash freeze when hit by an ice effect (anyone standing in them had to make a check to break free as a move action, or else they were immobile for 1 round), and

*icy pillars that could be smashed if dealt enough damage in a single blow, spraying ice shards into adjacent squares for some damage (effectively giving melee fighters a sweet short-range area attack in case they were immobilized by the freezing pools but still in a clever position).

So, what cool location-effects and stunts have you done, and how'd they work?

Oh and a funny one:

I had a badguy in 3E that fought the PCs in a big city. He had an escape route using a big tapestry that was hanging off a bridge above where a parade was.

The PCs beat up his allies and got a few lucky hits on him. Did more damage than I expected. So the guy runs, jumps over the railing of the bridge, and I roll the check he needs to make a "dagger slide" down the tapestry to safety (I think it was a Balance or Tumble check).

He rolls a 1.

Poor bastard doesn't clear his dagger from his sheath before getting all squishified against the ground.
