First, yeah, that's not RAW. Xanathar's is entirely optional. Second, even if you use that rule, you still don't roll if there's no meaningful consequence to failure and a poorly cooked meal isn't meaningful. Xanathar's doesn't override actual rules. You'd roll for that DC 10 if you were trying to impress someone by being able to cook a typical meal really well or if some other meaningful consequence was on the line. Right. You don't roll unless there's a meaningful consequence for failure, like a tire blowing out, someone stopping in front of you suddenly, a pedestrian stepping out in front of you, etc. Even then the DC for most of that stuff is 5 at most. The typical doctor never has an emergency like that and when the typical doctor does, he calls in a specialist who is not typical. You did in fact move the goalposts by adding in a change to the scenario that necessitated a specialist also being added in. Doctors are not the equivalent of low level. None of them are. They all have expertise and a bunch of level equivalents in their skill due to their intensive training. A basic nurse is probably +3 or +4. I simply corrected your False Equivalent. Perhaps pick a different profession that doesn't need to be that skilled. Of course it does. There would be no attempted organ transplants in a D&D world outside of a mad scientist scenario or something. Cure spells or regeneration would be used. Yep. And an unskilled person with a decent stat or skilled person with average stats here in the real world would have a lot of trouble with anything moderately hard. You don't roll for most things(routine for jobs) or else they are made easy by machines. Moderate to hard work takes very highly trained individuals who would be the equivalent at that skill of a much higher level PC and/or have expertise. You're as good as you should be in that situation.
