Yeah I know lame title. :)

What races throughout the history of the game have been able to create 1/2 races?

Elves + Humans
Orcs + Humans
Ogres + Humans
Dwarves + Humans
Demons + Humans
Angels + Humans
Dragons + Humans
Vampires + Humans

What else? (I'd like to stick to "official" races that have appeared in the past.)

In D&D, dragons can mate with everything. Even with insects...

What is meant to be a majestic and awe-inspiring beast has been reduced to pervert omniphiles...

Orcs and Ogres can mate to produce the ogrillon... That's when the daddy is an ogre, and the mommy is an ork. If the daddy's an ork doing the nasty with a mommy-ogre, the result is called an orog.
 

There are half templates for...

half-fey/Feytouched
half-troll
half-ithilid (well, an ithilid in a non-human host)
half-elemental/Genasi

Shifter (True Lycanthropes + human)
Changling (doppleganger + human)
Half-Giants (Athasian/Psionic)

In addition, there is Chaonds and Zenythri, which are descended of creatures of chaos and law, respectively. Their actual parentage is unstated.
 

I was rather under the impression that Genasi were half-genie, not half-elemental.

And there's no such thing as a half-illithid human--If you do the things you do with a monster to create a 'half illithid' monster, then you just get a standard illithid.

I think he is referring to the Half-Farspawn from Lords of Madness. I forgot what aberration it comes from though, so could be wrong.

Stick just to official races, or also to official rules as written?

I ask because there's a big difference. (and in what rule set can Humans and Dwarves interbreed? That's a new one on me...)

And even without going beyond the rules, just think of the genetics that went into producing the following races, and thus what they can possibly breed with:

- Centaur
- Tabaxi ("cat-man" from one of 1e's MM2 or FF, I forget which)
- Hobbit/Halfling (are Tallfellows descended from Elf stock somehow? And, what if anything can crossbreed with Halflings?)

And when you go beyond the arbitrary limits of the rules, you quickly wonder whether, if all those things can cross-breed with Humans, how much of a stretch is it to think they can crossbreed with each other? What about the half-races? What happens when a half-Dragon and a half-Elf reproduce - do you get a 1/2 Human-1/4 Dragon-1/4 Elf? You should...

In the initial list, the following are missing:

Deities + Humans
Dryads + Humans (though this only generates more Dryads, I think)
Nymphs + Humans (and Nymphs + Elves?)
Satyrs + just about anything that moves...
Trolls + Humans (I seem to remember this from somewhere - 2e? - could be wrong)
Trolls + Giants (giving the "Giant Troll" from 1e)

Kingdoms of Kalamar had a lot of 'em. I remember these three, but there are more:
orc + elf
gnome + halfling
hobgoblin + human

True, genasi are descended from genies, but half-elementals are descended from any type of elemental (including genies, sylphs, pechs, etc).

A "half-illithid" is when an illithid puts a tadpole in a non-human host (such as a giant, lizardman, or goblin). They don't become "true" ithillids", but so they are "half-ithilid, half-X". Its certainly not sexual reproduction, but it is another "cross" race.
