3.x and 4E, no

But in 2E, I used it a couple of times, usually in the last session of a game at the university to end a campaign. Draw till you die.

In one case, I used it in my face to face game, and the party ended up drawing 5-6 cards. One character got imprisoned, and one got rich, well could have but with terrible rolls on the gems, he got shafted rather severely. It was something like 20% of what he could have gotten. Another character got to fight death, but see the next story.

In one face to face game we had gotten so powerful that the death was a joke. Sure he did damage, but could not kill anyone. WE drew that card a could times and each time each character killed like 5-6 of their own deaths. It was hilarious. The fight drew the card and everyone else ganged up on the death to get heir own, which they fought pretty much alone till one was left and then everyone ganged up on that one to get their own. The DM, me, gave them a lot of xp for that.
 

I had one in 2e, I'd never heard of this artifact before (this was our group's first campaign, outside the DM) and so when Lazlo Panaflex, the planar hotstepper, asked us if we wanted to draw some cards that could grant us great benefits, or terrible penalties, we all said, of course, what's the most we can draw. The DM explained OOC for a moment that this was a very potent item and not to be taken lightly.

One player replied, "Cool, what's the most I can draw."

One guy drew 1 card, getting the Fates card to avoid one situation.

Another drew 1 only and got the Throne card.

I drew 3, First up...Ruin. Lost everything for my (2e) Paladin, magic plate and magic longsword. Bummer. Then I drew the Knight card, and finally Flames. Uh-oh, the look of evil glee on the DM's face was priceless.

Finally, our barbarian drew 3 also - he drew the Key, Fool, Jester, and Star. Lucky guy.

So, the keep turned into a fairly major part of our campaign as the deed that appeared was near the Cormyr area of FR and upon arrival it was in disrepair and had been taken over by a large tribe of hobgoblins, which took quite some time to remove. After tons of time repairing it, it eventually grew into a small town that is now a permanent fixture (and usual starting locale of any campaign) with our PC's from that first game as the leaders of the, now small city.

The best moment though came in the final battle, aboard the floating castle at the head of the undead army, against the devil behind the invasion. A Marilith. It was an epic battle, my fighter servitor fell in the battle, but we overcame the foe, driving it back into its portal. As it fell in, some....thing from beyond the portal lashed out to try and pull us in. Luckily, it mostly missed, except one hit that dragged the PC into the portal and sure death, then the portal deactivated, after we smashed something holding it open. The player was bummed, but then started laughing. "Hey, I've got that old card from months back to let me avoid any situation...I'd like to avoid having that tentacle hit me." So we all survived.
