Some of it was probably left out due to space, some of it was posted too late to be used or got buried in the thread and went unnoticed, and some of it the authors didn't care to use or couldn't decide how to make it work with current continuity. </p><p></p><p>At the same time, those threads have hundreds of posts and are hard to wade through. So... If I have re-posted something that made it into the book, let me know and I will have a look again. If I forgot something that did not appear in the book, please feel free to add to this thread. Sorry, but there was so much stuff to look at, I often did not bother to note who originally posted something.
