The template actually comes from the Dragon Magazine that featured creatures appropriate to the Return to the Temple of Elemental Evil campaign. If you look for the templates there, you'll find the CR alterations.

Dragon 283 from Jul2001. Theme: Halflings. Conveniently, I had the issue on my desk open after getting it out to look at that same article.

CR adjustments are: up to 3 HD, +1; 4 HD to 7 HD, +3; 8 HD+, +3.

I've used this template before. It's balanced ... if you apply it to a true elemental.

Apply it to an invisible stalker, and it is no longer balanced.
