Disarm questions..

Greetings all,
Just curious..

If you notice someone decked out in nice fancy-schmancy accessories in gear, you can make a disarm attempt to disrobe them. The PHB says that if its easy to take off just disarm, if its harder, pin, then disarm. My question is: If you disarm/pin-disarm to remove something, does it fall to the ground or do you grab it? If it goes to the ground, what are the circumstances for picking it up and putting it on? What kind of AoO are given, etc...

Using a greatsword w/ Imp Disarm atm...

If you disarm with a weapon the disarmed item falls to the ground. To pick something up from the fround is a move action that provokes attacks of opportunity.

If you disarm someone in a grapple you'd have to do it unarmed, which means that you end up with the item in your hand.
