Disease - A lot of questions unanswered

The Disease section of the DMG doesn't address a lot of information presented.

For instance, what's the formula for the disease's attack? In some cases it looks like Level +3/+4, but it's not clear.

More importantly, I'm not certain about the DCs of the endurance checks necessary.

Finally, do you get any XP for overcoming it?

This is one of those things they didn't give us rules/guidelines for. It appears that it goes off DMG 42. Improve is Hard. Maintain is Normal.

Do you mean "What do these do?" At the end of an extended rest, you roll an endurance check. If you beat the Improve DC, you move up on the ladder. If you beat the Maintain DC, you stay put. If you get lower than Maintain, you move down. Rinse and Repeat.

I'd say no for XP, most because it is mostly a passive thing. You make a few rolls, say your prayers, and you're done.

Then again... if it actually challenges the party, it could be considered it's own skill challenge, or even maybe a minor quest. In that case, they would get XP.

Basically, give 'em XP if you to.

They're supposed to be. Note that the errata has changed all those DCs, so maybe they match up with the table a bit better now.

I was referring to the DCs. For the most part, Improve is generally around the Hard DC and Maintain is around the Medium DC. It doesn't match up exactly because nothing in 4e does. Check out the MM.

As for the actual attack bonus, it does appear to be Level +2/3/4. Basically, set it to whatever you want. It's the same deal as when making monsters... just make it however you want. There are no hardset rules for creating them.
