"Demon lords" is usually just a naming of convenience for the most powerful ones, typically the ones that are unique beings instead of belonging to a monster entry; therefore, Demogorgon, Grazz't, Yeenoghu, Juiblex and Orcus are "demon lords" while Balors (more than one of which exist) are not, although they may be very close.In more concrete terms, you may not worry too much about any hierarchy when using demons in your campaign, since they chaotic evil nature is such that even if there was any hierarchy it would be very flexible and everchanging. If a demon lord remains as such it is only because he is powerful enough to survive and not be overthrown by any other demon.</blockquote>
