Has anyone used one of these yet? If so, do you have a template for a monster stat block? I don't really see one in the template or at least one that looks decent.

I think that covers what your looking for?

P.S. There is some question about whether or not those stat blocks would count as trade dress, so publish at your own risk. The excel template would be easy enough to modify though.

I used the LibreOffice template by Dale Robbins but had to create my own stat box format. Only real challenge was the ability table and that wasn't too tough.
