Expansion of existing combat rules, skills and class concepts

What ideas do people have for extra uses of skills, or more rules for combat and more uses for class specific concepts?

I was thinking for example, instead of always using the sneak attack opportunity for damage. A rogue or whichever class that uses a sneak attack, could forgo the attack and use an alternative "dirty trick":
ie.
- Well aimed kicks in groins causing ( or other painful areas) causing -2 to attack rolls for 1d3 rounds per sneak attack level
- Temporarily blindling ( sand in the eyes)
etc...

As another example, I think a lot of the bard's extroadinary abilities could be used for other purposes.

Expansion on criticals concepts. For example, when a player has the possibility for doing critical damage they can opt to peform a called shot, rolling again against a certain DC level for the type of shot + AC. ( I know called shots can be very powerful, having them available only during a critical and setting hard DCs though would make them less volatile.) The advantage of the called shot of course would be things like hampering movement, blindness etc.

Also, other ideas similar to "Help Another" or whatever its called would be a good addition to combat rules.
Anyway I'm blabbing on a bit too much here.
