Failed attempts to gain super powers

And there's at least one story which says that Dr. Doom was so eager to don his armor that he put on the faceplate while it was still white hot and that's the main reason his face is deformed.

To answer Bullgrit's query above, the scene you described is from Peter David's run on X-Factor in the 90's (Note: not the 2000's series featuring the private eye Jamie Madrox). It was just one of those hilarious asides David likes to insert in his work. Don't remember though what issue number it was, but I'm pretty sure it was during David's run.

Didn't Ted Sallis transform into the Man-Thing as a result of a failed attempt to reproduce the Super-Soldier formula that turned Steve Rogers into Captain America?

Ruins follows former Daily Bugle reporter Phil Sheldon as he explores a dystopian alternate Marvel Universe where, in his words, "everything that can go wrong will go wrong"; a world where the myriad experiments and accidents which led to the creation of superheroes in the mainstream Marvel Universe instead resulted in horrible deformities and painful deaths.
