1) Look at the difference between levitate and fly. Spells do exactly what they say; nothing more, nothing less. Levitate explicitly tells you about penalties for trying to attack while levitating. There are no penalties at all for spellcasting while levitating (unless you are making attack rolls) since the description doesn't mention any. Fly, in the other hand, doesn't mention any difficulty at all. There are, therefore, no penalties. 2) Yes, it is correct. Yes, you provoke an AoO. you can avoid an AoO many ways, primarily by making a 5 foot adjustment out of reach, or moving (not running) and then performing no other action. It does not have to be a double move, and yes, you can run while affected by the fly spell; you would move at 90 x4 (or x3 if you are encumbered or wearing heavy armor). 3) I agree with AuraSeer. Flyby Attack does not state that you do not provoke AoO, so unless you had Spring Attack as well, you would. I suppose that is not an explicit rule and that you might be able to take it but only use it when the prerequisite is met, but I argue it is definately inferred. 4) Yes. You could also use Shot on the Run for ranged attacks. 5) Yes. It would be called a charge, and would be resolved exactly the same: -2 AC, +2 to hit. You would move not less than 10 feet and not more than 180 feet (90 x2). You would have to move in a straight line. You would be able to make a single melee attack at the end of the charge action.
