Game Session SUrvey/Polls

I know in the past more than one poster has mentioned that they pass out surveys/polls to their players after each session or every few session or something to gain feedback on the game and see how people are enjoying the game and what suggestions/criticism they mioght have.

How many people do this? And does anyone have a set example of such a survey they can post or send to me? I'd much appreciate it.

After every session? No, not nearly.

Every once in a while, perhaps. WHen there's major changes in the player pool, or been a long-ish hiatus in the game, or if the DM is contemplating a major change, or has been doing some experimenting for a little while.

There is such a thing as asking for too much feedback, and over analysis.
 

Usually I send a game in-review e-mail to my players and they send feedback as to what they liked or, god forbid, disliked.

During my e-mail I like to critique my players from the previous game and any changes I see, especially alignment.
