Lords of Madness, page 9, first full paragraph, last sentence, says "Various forms of gates and other world-spanning magic permit a knowledgeable traveler to reach an entirely different world with a mere step."Just to be clear, this is under the "Alternate Material Planes" header, so between that and the preceding sentences, there is no confusion: a gate spell (or some form of it) does let you move from one cosmology to another.</blockquote>
