This is how I would do:
1. The smoke is heavily obscured, your'e effectively blinded.
2. The smoke could be quite hot. If so you take 1d6 fire damage each round, CON save vs. DC 10 for half.
3. Hold your breath, use the rules for suffocation. When you can't hold your breath any more you start roll CON saves every round vs. DC 10 or lose a level of exhaustion.
I would really like to use the poisoned condition, but it overlaps with exhaustion so there isn't any point in doing so.
