The Cult Leader who has achieved near-Demigod status (a la Thulsa Doom, in the movie, "Conan The Barbarian.") "The Secret Brotherhood" whose agents are everywhere, corrupting all known governments... The self-sacrificing hero who has been brainwashed by an artifact, and possessed by a Demon Lord (a la the hero from Diablo I.) The good arch-wizard who is being coerced / blackmailed by a Demon Lord who is keeping his daughter prisoner. The careless Demigod who accidentally loses his artifact in the world of men, and the fight to possess its powers and secrets starts a war between 2 or more nations... An evil god of a humanoid race witnesses a miracle, and has a change of heart (shift to good alignment)... Turmoil erupts among his followers. The ones who follow him become good-aligned, the ones who liked being evil begin to disavow this "false god," and begin to worship the "true incarnation," who is actually a Pit Fiend who has assumed the evil part of the deity's abandoned portfolio, and then there is the undecided faction who seek to discover the truth of the matter. (I used this one in my campaign, it worked rather well. I have attached the PDF to this post.) ...That's all I can think of, just off the top of my head.
