Hit points for templates (I.E. Ghost)

Let's say I am playing a fighter for 5 levels, at which point I have 42 Hit Points. During this level, my character dies but comes back as a role-playable ghost (using the template), do I immediately reroll all my Hit Points (5 levels worth) using a d12? Or do I simply use a d12 from now on, if and when I advance levels?

As a 5th level fighter your HP loks like 5d10+(5*con bonus). As a ghost your HPs become 5d12. I'd either make the character reroll all the hps or take his fighter hps, subtract out the con bonus, and then add 5 (the difference of an average roll on a d10 verse average on d12.
