Most illusions allow saves when "Interacted with".

When does the save come?

For alter self spells is it when they touch you or vice versa?
or When the start talking to you?
or When they first see you?

Same for ghost sound, how do you interact with a sound? Do Pc's/creatures get a save the second they hear it?

It's disbelief. If you don't believe in an illusion, then you get a will-save. The problem being, that sometimes it is very difficult to spot the illusion from the reality.

Alter Self: if the person you are talking to realizes that you are not who you are claiming to be then he gets a will-save.

There are several clues, which may or may not give away an imposter using a Alter Self or Disguise Self spell. These are usually governed by a Spot check. Now, usually when a person first approaches a character shrouded by one of these illusion spells the DM makes a Spot check in secret to see if the illusion is fooling the observer.

When the person shrouded by an illusion and the observer begin to interact, then there might be another Will-save. This is governed by the observers suspicious nature. If the observer doesn't show any suspicioun against the shrouded character, then there is no Will-save. If, however, the character is even slightly suspicious of the shrouded character's real personality then he gets the Will-save.
 

I think that it's really DM's job most of the times, to adjudicate when a ST is appropriate depending on the circumstance.

For example, when someone is using Alter Self to look like your girlfriend, you may notice from what she says or the way she behaves that she really isn't. The DM may give you a ST after having spoken with her for a while and a successful Sense Motive check (possibly repeated later).

Against an illusion which fools the looks of an object, you are interacting with the object if you for example touch it, and can feel that the look doesn't match with the touch.

That is for example typical if you are fighting against an illusionary foe, which has no substance: if you hit him or if you are hit and don't feel the hit at all, that's definitely an interaction enough to get a ST.

It could be more difficult if the illusion is a sound, or a smell.
 

I agree about alter self with the will save. Unless you had a reason to suspect... no save.

But what about using major image to create a cart moving down a road?

It looks and sounds like a cart, but does it travel over the ground 'right'/are the noises right. How well does the caster know carts? How familiar is the observer? Is it a Spot Check, Knowledge: Carts, Handle Animal, Will save?

IMO, it's quite 'GM-call-tastic!'
