These are monsters I created for an adventure two years ago, before I even knew of the impending Book of Vile Darkness. I've recently tweaked them and decided to share them. Be warned: these monsters are not for the sqeemish and the nature of their background may not be suitable for some people.

Innocence Lost, or the Innocent, are the unfortunate souls of murdered children, often those sacrificed by the mad and utterly evil servants of dark gods and fiends. In Undeath, the Innocent witlessly seek revenge against all adults and other guardian-figures (like pets, soldiers, etc.) for not protecting them. They often haunt the areas near their deaths, although the Innocent Lost who were five years old or younger often (75%) return to their families, killing all of them.
The Innocent appear as hazy, transparent images of their former selves, often bearing the gruesome wounds that led to their deaths. They are constantly crying out for love and aid, tears streaming from their eyes.
Combat
The Innocent swarm in, their arms outstretched, wailing and crying for safety, love, and affection. Whether or not their Wail works, they attack with their touch, hoping to drain the lives of their victims. They rarely retreat and often embrace opportunities to be destroyed by divine means.
Touch of Innocence (Su): Innocent Lost seek the warmth and comfort they were robbed of during the last moments of their lives. As such, they often energy drain opponents rather than take their Constitution (they cannot do both). Living creatures struck by an Innocent?s touch of innocence attack receive one negative level. Interestingly, Innocent do not seem to receive any meaningful benefit other than the opportunity to touch a living creature. The Fortitude save to remove a negative level is DC 14.
Wail of Sadness (Su): As they enter combat, the Innocent issue a wail of sadness, often begging for cherished toys, their parents, or protection; they may Wail 1/day. The Wail affects all living creatures within 30? as if they were struck by a charm monster spell cast by a 10th level sorcerer for 1d10 rounds. Those who fail a Will DC 16 will seek to protect the Innocent, even allowing the spirit to continue draining them.
Embrace (Su): If an Innocent succeeds in two successful consecutive touch attacks on the same opponent, it may attempt an Improved Grapple with a +5 profane bonus to its check as a free action. If this occurs, the victim automatically takes an additional 1 point of Constitution damage.
Incorporeal: Can be harmed only by other incorporeal creatures, +1 or better magic weapons, or magic, with a 50% chance to ignore any damage from a corporeal source. Can pass through solid objects at will, and own attacks passes through armor. Always moves silently.
Turn Resistant (Su): Due to their being the souls of children, even the most dedicated priests are stunned in dealing with Innocence Lost. Innocent receive a +5 circumstantial bonus to their turn resistance.
Undead Traits: Innocence Lost are immune to mind-affecting effects, poison, sleep, paralysis, stunning, disease, death effects, necromantic effects, and any effect that requires a Fortitude save unless it also works against an object. It is not subject to critical hits, subdual damage, ability damage, ability drain, energy drain, or death from massive damage. Innocent Lost cannot be raised, not can they be resurrected. Innocent Lost have darkvision (60').
