I have implemented a "fail by 5 rule" for all my skills. If you fail by more than 5, you completely fail. If you only fail by 5 or less, you partly fail. I'll use Jump and D&D 3.x's rules as a skill to illustrate. You need to jump a 15 foot gap. You get your DC, roll dice, add skill ranks but you fail by 1. This result would be the same as hitting the edge about at the waist. For my game's rules, you would immediately enter a similar state as Combat. Time slows down, 2 second increments, and be in the process of Falling. You would have to very quickly state "I'm going to use my Climb Skill to stop from falling!" If you hesitate or say "awe shucks" you miss your opportunity to save yourself. Now because you partly failed, you get a chance to save yourself, but at a cost. Back to D&D, you would have a DC to beat to climb yourself up. For my game that DC would be 5 points higher than normal. The increase in difficulty is to simulate the predicament you're in under less than ideal circumstances. You may attempt any relevant skill as many times so long as you don't fail by more than 5. Each time you retry a skill during this time the DC increases by 5 more points. Now since you're in a "combat" state, anyone may attempt to help you. You use Combat rules on movement. Then other people can use Aid Other to give a bonus modifier to save you. I think this "fail by 5" rule is pretty sweet and I have it for all my skills, even the ones contested by other players or NPCs
