Ruled by a council of generals (rumored to be undead) and their factional mercenary armies, heavy mercykiller presence, slavery to an army is a common punishment for law infractions, heavy planetouched influence.
There is some info in the 2e guide to the outlands and the 3e module Lord of the Iron Fortress though mostly scant references.
I reimagined it for my own game to be factionalized between militaristic humanoid and planar LE/LN oriented races: Hobgoblins, duergar, asherake (from Bastion Press' Complete Minions) fire giants, azer, bladelings, and tieflings with a smattering of others. Note the absence of humans. I also had an army of a half a million religiously fanatical orcs camped outside its gate walls and had a city faction of goblins mysteriously dissapear in the recent past (dragged off to the Oathbound Arena setting by Bastion Press).
