Min/Max: Polymorph and Shape Change

Ok, all you min/max'ing fiends. Someone must have done the legwork on this already. What are the best "polymorph" and "shape change" types?

I'm especially looking for those that have the best impact on:

Initiative
Attack Bonus
Armor Class
Attack Routine
Attack Routine/Weapon Damage
Tactical Advantage
Damage Avoidance
Damage Reduction
Immunities
Movement

Also, anyone get a "DEFINITIVE" ruling on what carrys over with you in the new form, and what doesn't?

Which of these would carry over if you were altered/polymorphed?

Enhancement from spell (Bull's Str)
Enhancement from item (Belt of Str)
Inherent to Str (from book)
Size bonus from Enlargement (Enlarge Person)
Morale bonus to Str (Rage spell)
 

AFAIK, all but the size bonus, unless your new form is also humanoid (altho even then it might not).
The belt is only useable, if the new form can wear it, tho.

How does one determine what items a creature can and cannot wear? Can a druid wildshape into belt-wearing bear or wolf? How about an eagle wearing a ring on one of its talons? What's a good way of determining where the limit is?

Marilith and titan are both excellent forms. ;)

Druid in my game has been stomping over every encounter in Marut form. Christos.

Construct (not living, immune to mind-affecting), DR 15/chaotic, one fist blinds, the other deafens.

If it wasn't for dispel, disjunction, and mace of smiting, I'd end the game right now.

I'd go by common sense there, but mikebr99 posted a good link about the subject, which could be useful as a guideline. :)
