Basically, the worse someone does on a save, the worse they get affected (with 3 or 4 degrees of failure being the amount needed to take someone out). Each degree of failure basically gives different penalties or status affect. So yes, there is definitely a death spiral in M&M 3e.</p><p></p><p></p><p>Unfortunately, since there are no SRDs online, I can't point you to a link for a free copy of the rules.
