Avernus is nominally ruled by Tiamat, the Chromatic Dragon. The mother of all evil dragonkind holds Avernus through the support of Asmodeus, but her actual authority extends only as far as her physical reach. Most of the inhabitants of the plane avoid her, not wishing to serve as her meals.
The Chromatic Dragon spends most of her time sprawled in her lair, the great caverns known as Azharul, "The Dragonspawn Pits."

Tiamat's Lair is simply called "Tiamat's Lair". It's located on Avernus, the first layer of Baator. The mountain is unnamed. In fact, Avernus is covered in mountain ranges that are not named.

Avernus is not Tiamat's realm, it's just where her lair is located. The Arch-Devil Bel is the Lord of the First and rules over Avernus. Although, other deities have realms on Avernus. I suppose you can call Tiamat's Lair a realm.

More information on Tiamat's Lair can be found in the 2e Planescape adventure Fires of Dis. It's a great adventure and one that never gets the attention it deserves. :cool:
 

I think there's an edition problem. In 2ndE, Tiamat has her lair in Avernus. However, in 1stE, it is stated several times that Tiamat is the ruler of the 1st Hell.
Fluff is not the same between editions. Some things always change ^^
