The caster creates a double of himself at a point within the range of the spell. This double has the same AC, movement etc. as the caster but is a mindless clone that is controlled through a mental link. When the caster stops to concentrate on the double, it explodes, spreading a rain worms and small crawling insects within a circle of 10 feet radius. Everybody within this radius is attacked with a ranged touch attack based on the base attack bonus of the caster and a dexterity of 20 (+5). Those hit have a vile insect creature attacked to themselves, which will bite them until slain (the first bite occurs immediately,before the character can react). The other insects diappear immediately. Dealing damage to the bloated double in any way causes it to explode. A spot check vs DC 10 + caster level reveals that the double is lightly bloated as opposed to the original.

1) AC of 14, not equal to caster, still modified for size
2) equipment useless, non-magic copies, unable to let go of them, let alone use them.
3) HP of 1 (he is a vermin baloon)
4) able to move at your normal (for your race) movement (being a monk will not allow you to make a faster moving vermin baloon)
5) It is incapable of making attacks, casting, or speaking intelligibly.
6) Some way to realize something wrong, perhaps?
7) Your description makes the insects sound small - but 1d4 / LV is huge (that is one mean bug). Perhaps target covered in LV+d4 insects, and take their # in damage each round, but subtract their natural and worn armor value from this total. The insects die off, reducing their number by 1/2 each round. A creature can crush the insects on them as a full round action, destroying 1d6 insects each round. Another creature can aid this, killing an extra 2.
8) insects die / get off target in smoke or in liquid.
9) this is per a swarm, do not roll to hit (a 20 Dex thief in a bee swarm will still get stung).
