Conditions are listed and defined in appendix A. Any other "conditions" we might talk about could therefore be considered house rules.

I suppose you are right. If you give the tag condition to "dying" that is a house rule.

I think that hair is split so thin that it hardly makes sense. But I can at least see how someone's logic might interact incorrectly with that.
