Yes. At first the alignment change only occurs when he changes form involuntarily. If he ever voluntarily tries to change, he has given into the curse and the alignment change becomes permanent. No, although it might be possible to use magic to determine if he's been afflicted or not. No, but nothing stops you from using belladonna and doing a heal check just in case, as a preventative measure. No, if he chose not to be cured I would rule that he has given into the curse and is now an NPC. Inflicted Lycanthropy is supposed to be a curse, not a benefit. If you ever give into the curse, it's not really your character anymore, but the curse that is in control. Hence the alignment change. The other possibility is that the DM creates a prestige class equal to the ECL adjustment of the lycanthrope, and allows the character to gradually how to control the curse without losing his alignment. I would put some heavy role-play requirements on this, such as seeking out someone who can teach the class. Until they learn control, they will be plagued by involuntary changes, and may be held responsible for any crimes commited when they undergo the change, as they have not sought out a true cure. I don't really reccommend it, but it's one way of trying to keep it balanced.
