Two options on the hand thing - give him a magical/steampunk hand that actually works like a hand. Let him meet a cleric that can cast regenerate. You are using 3.0 right? Let him keep Monkey Grip so he can swap between two-weapon use or dropping his off-hand weapon and getting 1.5 x Str bonuses when he hits a creature real hard. EDIT: Commentary on your brother. Maybe you can get him to stop playing? Sit down at your computer and put together a fancy certificate with some blank spots for the player name, character name, DM name and the date. I would suggest making it ~ 1/2 page in size. Print it out on some nice paper. Cut it out and stash it near the back of your DMG. Setup some fairly improbable situation that he will come up with some wacky solution for. When it happens, just stare at him for a few moments with a dumbfounded look on your face. Slowly change it to awe. Shake your head slowly as you pull the certificate out of the back of the DMG. Start filling in all the blanks, sign it and then hand it to him and ask him to sign it. Explain that he just won the game and he gets that certificate as a reward. Now, since he won the game, there is no real sense in him playing except with people who have also won the game. You will keep playing with the other guys and maybe, just maybe, someday, they will win the game too. *shrug* Maybe it will even work.
