Cool, this idea is very close to what I run in my own game. I have run 2 different campaigns in that world - one lasting 6 months and the other about 5 years that both eluded to many of the same things in your world. The Basic advice is already covered (and I am sure you were aware of). Let the plot and action flow rather than being set. Forshadow the Far Realms with monster and such. Don't force the players down the path if they do not want to head that way. I would work on the Lich Kings goals? There are all kinds of goodies buried in there. He could help and hinder the PCs, seeking to bring others into conflict with the Far Realms since he is slowly loosing. He might want the PCs to bring the Mighty Legions of Krall against the Far Realms. He could be somekind of living key, keeping the gate shut and want one of the PCs to replace him? He could be corrupted by the insanity and know that he will shortly turn condeming everything he ever loved - both wanting to destroy the PCs and wanting to be destroyed by them before he is lost. Next...Evidence of what occured? How about making a city in somekind of temporal haze that is suspended half in the far realms? They open a portal to it and it functions fine but the stones on that end are missing and they will need to rerieve it before t-porting out? Uncovering agents of the far realms who have managed to maneuver a church into confronting the lich king. By this point the PCs should hate him... but now they realse that something wants him dead.....something far worse than him and how do they get the paladins of the church to allow this lich to continue? Gates of Firestorm Peak could play very well into your game. That's is about it off of the top of my head.
