Charges are also an exception, since the combatant with the longest reach/weapon will strike first.</p><p></p><p>For a definitive, by-the-book approach to 1E combat, check out <a href="http://members.cox.net/dmprata/ADDICT.pdf" target="_blank">DMPrata's ADDICT document</a> (link is to a PDF).
