Mostly role played it all, although there were sometimes rolls to make. The chance for a relationship was an option for random occurances in the magical school game I ran (which was much more of the fantasy high school simulator than I was expecting). In some other games PCs made the decision for such and would have to make some rolls to initiate. From there it was mostly role play: Give the NPC a full background, personality, and agency. Give them some desire or demand that the PC would have to react to. The relationship required effort and time on the PC side to maintain. ETA: In the magical school game, three of the four PCs ended up with a SO. The forth was a naive ace who wouldn't even get the rude double entendres the others would shoot back and forth (remember how I said it ended up more a fantasy high school simulater than I expected? Primary spell choice was based on if they could use the spell to play pranks on each other). Most romantic parts were done OOC usually before game. Most notable when all the players sat around and asked each other "Do you or not? and do you tell the rest of us?" In the end, one character and their SO were still together and in the party at the end. One player decided to retire her character and bring in a new one, and her character ended up going off with her boyfriend to live with them him after they graduated and later married asa good end to her story arc. The last one was dating the Draco Malfoy stand in as the stuck up noble's son (the SOs were determined at random from a list of other students). She eventually said it wouldn't work out due to his responsiblities to his family and called it off. They got back from an adventure later and I swear there was a OOC uncomfortable pause and hint of regret in her greeting when they met again and he introduced his new girlfriend and fiance.
