I'm willing to say what it looks like: the SW quadrant of the city was become a stream and a waterfall. An island splits the stream, and bridges lead between the island and the rest of the arc of the city.
There's also another section of the city that's been partially melted on the NE side, I believe.
From that, you should be able to create your own map.
As for a scan, you're on your own. The post-campaign map is one of the selling points of the HC. If you want the official map, buy the book. You don't have any right to the map simply because you bought the Dungeon issues.
