D&D 5E Sheet for Character info for DM to use??

Hello folks,

Does anyone have a link to a decent single page sheet that will hold the core character information for me to use while running the game?

I am looking for something that has max/current h/p, passive perception, trained skills, current a/c, etc....

Any help is appreciated. I could make something up in publisher but thought if someone already has something then why invent the wheel.

I can't work out what the three boxes along the bottom-left, with 'AC', 'hit points', 'class & level', are for (next to 'notes'). Anyone have any ideas?
 


This is a neat sheet. I used to make my own spreadsheet to keep track of such things and list their kills for easy exp calculations. But I must say I found an app that can be used with iPhone or iPad (easier on iPad with larger screen). It's called "d20 DM 5". This app allows you to actually include the entire character sheet for all party members and monsters. You can easily build encounters then it will auto roll initiative and keep track of rounds, hit points current and totals, current conditions (like poison etc), space for notes, and even rolls the dice for the creatures to hit and their damage if you want. It even has the basic game monsters pre-built into the app so you don't have to create anything but the PC's and start playing. It handles everything. If you pay the small cost they ask (stars off FREE for all described here so far) then you are given access to much more such as basic rules, equipment and armor lists costs and everything! It's like having the PHB and MM in your phone! Way cool if you have an apple product. There may be an android version as well - not sure. More than what the post was asking about but it does keep up with the PC's if that is all you wanted to do with it.
 


Here are two I made last night. My goal is to have one or two pieces of paper with information I don't want to keep asking players about (e.g. personality), and information I use for secret rolls (e.g. passive perception, passive investigation, who can understand what language).

This is definitely a work in progress. :cool:
