Starting a Campaign

So, GMs:

How do you get ready for a new campaign? How much prep work do you do, and what KIND of prep work is it? Do you plan out adventures in every detail, or do you like to improvise things as the game moves along? How much setting information, if any, do you give the players if you're not running in a published setting?

As I gear up for running a Gamma World campaign, I'm finding myself getting bogged down in the details. The last time that I ran something I didn't plan ENOUGH, and this time I seem to be planning too much. Where's the happy medium?
