Anyone out there run a campaign that doesn't use the teleport spell or any similar magic, and if so what have you done (if anything) to replace it.

IMC, Teleport is limited to 1 mile/level, and Improved Teleport (7th level) is limited to 10 miles/level.

Gate, Teleportation Circle and mundane transportation must suffice.

The players in my campaign dont have access to teleport or any similar spell, because it's heretical. I haven't replaced it with anything. It has been interesting, with them actually travelling places rather than "poof we're there." It's making me consider an expensive material component for teleport next time I run a campaign with it. Expensive enough that you don't use it to get everywhere, but cheap enough that it's still a useful spell when you need it.
 

Teleport doesn't exist IMC either and this was done to promote the use of flying mounts (and some very, very swift land-based mounts) at higher levels. The most powerful of Druids in our setting however can create gateways that travel along the "roots" of the new World Tree (in a metaphysical sense) connecting far off regions. They are very, very rare and difficult to make however and the Druids take great pains to hide them from the world at large.

My campaign world is enormous- it takes place on the inside surface of a bubble of air in an infinite ocean; the bubble is about 780,000 miles in diameter. So when I started it up, I decided that I'd limit teleport to 100 miles/level and teleport w/o error to 500 miles/level, and succor (nowadays it's called refuge I think) to 1000 miles/level.

It's a long enough distance that the teleport spells still let the pcs go just about anywhere they've been before, but not anywhere in the world, which allows me to keep lots of unknown things out there.
 


iMC, teleport is only available to certain NPC mages who took a wayfarere-like prestige class without spell progression. There are some ancient portals around for faster travel, but they usually only work under certain conditions - a.k.a. DM's choice. I don't have flying mounts available, so people use ships whenever possbile, or caravans/wagons.
