Kesh did a very good job of explaining what was going on, though to clear things up a bit I'll add that as he said the first program was a Utopia that was rejected by the human mind, the second matrix program was more more like the one we see, but also failed. After teh second matrix the Oracle came up with the idea of giving the humans some level of choice in their lives that mostly fix the problem (i.e. the 99.9% of people accept it, with the 0.1% being what necessitates the One protocol). Well based on what is said in the movie, in particular the little exchange between the Oracle and the Architect at the end of Revolutions, I think it is clear that it was the Oracles plan and that the Architect was in on it from the beginning (of this cycle that is). So how things turned out was no accident, and that was her hope all along, though she had no way of being sure if it would actually work out right. It is also this scene that makes me think Neo isn't really dead and pretty much clinches that he is the One in each cycle. As for Smith, the Architect told Neo that he was the "sum of a remainder of an unbalanced equation," so if Neo is one, you would subtract one to balance things out. So, Smith's existence is critical in bringing true balance to the Matrix that the Architect's solution just didn't do. Apparently whenever he encounter a proble with the system he found it easier to just reboot it than actually fix it.
